This is the membership agreement. I would download this document and the Bylaws first and give then a good read over. There is a great information link with further reading about the current state of cannabis medical use & how being a member of a collective keeps you with in the boundaries setup by the law. http://norml.org/
a “collective” under the California Compassionate Use Act of 1996, as amended from time to time (the “Act”). The Collective may act only as permitted by the Act in providing a means for facilitating or coordinating transactions between its Members and will not under any circumstances purchase or sell marijuana to a person who is not a Member of the Collective. The Bylaws are a set of rules such as stated above that govern the actions and activities of the members. It's a good read if you plan on being a member.
Once you decide to become a member and you have found a doctor he will write a prescription that will enable you to use cannabis for medical reasons. At this point he will provide you with documentation a certificate and a Identification card. This verification form will provide us with all the information we need to register you as a member of the collective so that we can start providing you with your medication.
Please email these completed forms to and any contact you have for Bill to"
