Patient Information

As Patients you can join our collective and take advantage of all the products and services offered by our collective to our members. Your membership will be valid as long as your doctor's recommendation is. Once you have joined you will not have to provide anything but your name when ordering and your ID for identification verification at delivery.Joining our collective and placing your first order is easy, just follow the following steps to get started:

Agree to our Terms of Service

Evaluate if our collective is right for you. If you cannot agree to any of our terms but would still like to join, DO NOT AGREE. Please email us with your questions or concerns and we can work on a solution. If you submit your information into our patient form you are ABSOLUTELY agreeing to ALL the terms below. To fill out our patient form please click on Collective documents in the page list.

Completely fill out our Patient Form

Fill out as much information as is needed to verify you with your doctor. If you are not sure, please answer every question. Leave us a note if you are waiting on the next batch or want to be notified when a certain product is available. Please make sure to agree to each term at the end of the form or you maybe asked to fill out the form again. If you need to place an order for the same day please call us to start a rush order.

Prepare for your first Delivery

Have a color copy of your recommendation and ID/DL and the originals in order to complete the membership application. After you have signed and filled out the patient membership form and the terms and conditions you will have officially joined our collective and will only be required to show identification at delivery.
